5th and 6th Grade Tackle Football Registration
 
There are two parts to registration. Add which activity your child(s) will be participating in. Then Fill out the Registration/Consent Form. Your child(s) is not fully registered until both parts are complete.
 
Note: A physical is required to play 5th/6th tackle football.  You can print it from the bottom of this registration page. Physical form must be turned in by/on the first practice.
